Decision Making Toolkit

The Decision Making Toolkit produced by the Council For Disabled Children is a practical guide to support:

  • social workers
  • health practitioners
  • school and college staff
  • parent carers
  • families
  • and anyone working directly with children and young people with special educational needs and disabilities (SEND)

It is designed to be used in partnership with young people to:

  • support them to make their own decisions
  • participate as fully as possible in decisions made on their behalf

It includes a template which practitioners can use to support a young person who lacks the capacity to go through a best interests decision making process. This is based on the best interests checklist in the Mental Capacity Act 2005 code of practice.

SKILLS UP

Skills Up is a new support service in Halton that offers outcome focused activity sessions to clients with additional needs

  • Our sessions are tailored to meet personal needs and allow clients to make progress towards their targets, in creative and meaningful ways
  • Activities enable clients to gain skills and knowledge in areas such as independent living, community inclusion, health and preparation for employment, supporting them to reaching their potential
  • Sessions are delivered by former teachers, with enhanced DBS checks and relevant training in safeguarding and first aid. Sessions are charged at the Direct Payment Agency Rate
